JOB SUMMARY:   
The Family Nurse Practitioner (FNP) provides comprehensive, patient-centered primary care services in a Rural Health Clinic (RHC) setting. This role emphasizes preventative care, chronic disease management, acute care treatment, and health education for patients across the lifespan. The FNP works both independently and collaboratively with physicians and the interdisciplinary care team to ensure high-quality, accessible healthcare in an underserved rural community.

            Responsible to: Rural Health Clinic Director
            Positions Directly Supervised: None
            Regular Contact with: All Rural Health personnel, Physicians, and the public.

ESSENTIAL JOB FUNCTIONS:
  1. Perform comprehensive health assessments, including history-taking, physical exams, and risk assessments for patients of all ages
  2. Diagnose, treat, and manage acute and chronic conditions
  3. Order, interpret, and evaluate diagnostic tests such as labs, EKGs, and imaging
  4. Prescribe medications, durable medical equipment, and therapeutic interventions within scope of practice and state regulations
  5. Perform common in-office procedures depending on training and office needs
  6. Collaborate with clinic physicians, specialists, behavioral health providers, and ancillary services to coordinate comprehensive care
  7. Make appropriate referrals for specialty care, community resources, and social support services
  8. Communicate test results, treatment plans, and follow-up instructions clearly to patients and families
  9. Document all patients encounters accurately and in compliance with Rural Health Clinic (RHC) and regulatory standards
  10. Participate in community outreach efforts, health fairs, and public health initiatives important to the rural population
  11. Support health literacy and advocate for underserved patients facing financial, transportation, or social barriers
  12. Follow RHC-specified operational and billing requirements, including CMS documentation and supervision guidelines
  13. Participate in quality reporting programs or other clinic-required quality measures
  14. Maintain knowledge of state Nurse Practitioner Act, federal regulations, and evidence-based clinical guidelines
  15. Uphold patient privacy and compliance with HIPAA standards
  16. Adhere to all clinic infection control, hazardous waste, and pharmacy protocols
  17. Maintains a safe, healthy, and therapeutic environment for patients, staff, and visitors
  18. Participate in a periodic review of the patients’ health records as part of performance and quality improvement
  19. All other duties as assigned.

QUALIFICATIONS:

Education and Training:
Must be a Licensed Nurse Practitioner in the State of Louisiana. CPR and ACLS certification are required.

Work Experience:
Experience desirable but not required.

Physical Requirements:
  • Ability to move about the office and facility to perform job functions and provide services as well as access various departments and patient care areas.
  • May remain in a stationary position for extended periods of time.
  • Perform manual dexterity tasks such as taking vital signs, administering injections, and utilizing medical equipment
  • Constantly position self, such as bending, stooping, etc. to maintain and access office and medical equipment and provide patient care.
  • Ability to work in a fast paced stressful environment
  • Operates a computer, office, and patient care machinery.
  • Ability to effectively communicate with patients, visitors, coworkers, etc. via phone, in person, including the ability to clearly discern and respond to verbal inquiries, and exchange accurate information through conversation
  • Ability to observe details at close range.
  • Frequently move boxes, equipment, etc. weighing up to 20 pounds across the office for various needs.
  • Frequently assist with pushing, pulling, lifting, transferring, or repositioning patients, up to 50 lbs
